Example scenario for Social Workers involved in SUDI (time ordered flow chart)

Social work will be informed by Emergency Department staff of SUDI.

  1. The Emergency Department staff will make contact with social work as to whether the deceased infant, or any child within the same family, is or has been known to social work.

Social work will check relevant records.

  1. Social work will liaise with relevant hospital and primary health colleagues, police and child protection health team, as appropriate.
    1. Initial checks should inform social work staff of any potential child protection issues and provide useful background information prior to gathering further information from staff and parents. Social work staff will utilise the children at risk register and examine any previous concerns as to the well being of the infant / other child in the family and relay such information to the relevant Emergency Department staff in confidence.

Social work involvement required.

  1. A social worker will be assigned to the family if there is confirmation that the family has existing or recent dealings/contact with social work.
    1. If the family is not known to social work and such involvement is not required, then primary support will be given by health workers and the police as appropriate. However, if other involved agencies believe that support from social work would be of benefit, then social work will continue to be involved.

Healthcare Improvement Scotland receives confirmation to co-ordinate a SUDI review meeting.

  1. Healthcare Improvement Scotland receives confirmation from Area Procurator Fiscal that it is appropriate to co-ordinate a SUDI review meeting.
    1. Healthcare Improvement Scotland contacts the professionals involved and a SUDI review meeting is arranged. Ideally, this meeting should take place approximately 16 weeks following the death and will involve: pathologist, identified paediatrician (or SUDI), GP, health visitor and social work (if already involved with the family).
